Shift: 8 Hours - Day Shifts

Job Schedule: Full Time

Weekly Hours: 36.00

Salary Range: $32.00 - $48.00

  • Collaborate with surgeons, transplant coordinators, social workers, pharmacists, dieticians, and financial services.

  • Engage in a mix of medical assessments, logistics, and communication, making each day unique and fulfilling.

Please note that this position requires being on call approximately one day a week and every seventh weekend. Clinic hours are Monday through Thursday and Friday.

Job Responsibilities

As an RN Transplant, you will:

  • Coordinate efforts between the transplant center and major patient referral sources (e.g., Dialysis Units and Nephrology Clinics).
  • Assume a case manager role to ensure timely and efficient transplant education, work-up, updates, and activation for transplantation.
  • Manage a designated caseload of patients throughout the pre/post-recipient and pre/post-donor transplant process.
  • Coordinate referrals and address any roadblocks in the transplant workup process.
  • Provide educational programming for patients, families, and staff.
  • Ensure compliance with policies, procedures, and regulatory requirements.
  • Offer outreach services to regional dialysis and nephrology programs.

Qualifications

To thrive in this role, you should have:

  • A graduate degree from an accredited nursing program (AACN, ACEN, NLN CNEA).
  • A Bachelor’s Degree in nursing (or in progress with completion expected within five years).
  • An unencumbered RN license from the State Board of Nursing where you practice, or multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • A Minnesota RN license within six months of starting the position.
  • Required department-specific competencies and certifications.
